Team Select Home Care recognizes that leaving a hospital, facility, or another home care agency can be an overwhelming experience. Before your loved one discharges, or services are transitioned to Team Select Home Care, our compassionate team will meet with you at the hospital to coordinate your child’s care at home. After the initial meeting with your family, Team Select Home Care employs a collaborative and coordinated approach to creating a care plan that is specific to your needs. Your family can rest assured, knowing our highly experienced clinical team is available 24/7 to help guide you through this transition.

A Plan For Your Care

After the initial meeting with your family, Team Select Home Care employs a collaborative and coordinated approach to creating a care plan that is specific to your child. Fellow providers of your child’s care will be involved every step of the way to ensure the transition and duration of your child’s care is seamless and efficient. This collaboration creates a unique situation in which the care plan acts as a guide for your child, your family and the essential providers of your child’s care.

You and your loved ones are at the forefront of all communication with fellow providers and carriers of your child’s care plan. Team Select Home Care understands that a collaborative approach ensures quality care as you begin services with our agency. We understand that your child’s safety and health is of the utmost importance.

Parent CNA Program

What is a CNA? A CNA is a Certified Nursing Assistant and you can become one!

The state of Colorado supports parents or other family members, licensed as Certified Nursing Assistants (CNA’s), to be reimbursed through a licensed home care agency to provide CNA level care through Colorado Medicaid or Colorado Medicaid Waivers. Please contact your closest Team Select Home Care office to find out details on how you can become a Relative/Parent Provider, even if you don’t currently have a CNA license. You can find more information about the program here.

If you are a parent, and your child is eligible for home care services, you can become a Certified Nursing Assistant and be employed by our agency and earn income for providing care for your child. This is a wonderful exemption that the State of Colorado allows. Please contact your local Team Select Home Care office to discuss further.
