Team Select Home Care offers personalized home health care services to patients in the comfort and privacy of their own home. If you are recovering from an injury or illness, have had a recent hospitalization or inpatient stay, our home health care services can assist with your recovery and rehabilitation. Our goal is to assist our patients in getting back to their highest level of functioning independence.  If you meet Medicare criteria, Medicare will pay for 100% of home health care costs. Team Select Home Care will coordinate your Medicare Services and bill Medicare for you.  *Please note that services provided may vary by location.

Physical Therapy

    What is Home Health Care?

    Team Select Home Care is a state licensed, Medicare and Medicaid certified agency. We deliver exceptional professional medical care in the comfort and privacy of our patient’s home. By law, the patient has the right to their choice of provider. To qualify for Medicare Part A benefits, the following criteria must be met:

    • Patient is homebound*
    • Patient is currently under the care of a Physician
    • Services are reasonable and necessary
    • Care is intermittent
    • Services are skilled, reasonable, and medically necessary
    • Patient has had a face to face encounter (FTF) with their physician either 90 days before or 30 days after the start of home care services.

    *The term HOMEBOUND means, “a condition due to illness or injury which restricts a patient’s ability to leave home without assistance. Absences from the home are infrequent and of short duration.”

    What is Home Health going to cost me?

    If eligible, Medicare covers 100% of your home health services.

    We coordinate your Medicare insurance coverage and bill Medicare on your behalf.

    Eligibility for Home Health Care is determined by the following:

    Patient must require intermittent skilled care, physical therapy or speech therapy.

    Patient must meet home bound criteria defined by Medicare guidelines.

    Patient must currently be under a physician’s care. All home health services must be ordered by a physician.
